1/30/2024 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Jeremiah was not in a good mood, indicating he wasnât going to enjoy the session today. Jeremiah continues to express and elaborate on the negative relationship between him and his mom. Jeremiah continues to state that he doesnât want any additional assistance or support with his behaviors or feelings. Jeremiah was receptive to the idea of practicing breathing technique and meditation strategies at which mother reports behaviors last up to 20 minutes in duration. Behavior: Jeremiah continues to struggle with being able to manage his behaviors at which mother reports when he cannot have his way, he will lash out on everyone in the home with behaviors lasting up to 20 minutes in duration. Intervention: QMHP-C prompted Jeremiah to refrain from impulsive, hyperactive, defiant, and aggressive behaviors especially running, refusing authority instruction, and interrupting others. QMHP-C utilized behavior modification strategies including removing Jeremiah from the stimulant environment in efforts for him to reduce inappropriate behaviors. QMHP-C monitored effects of meditation and coping practices daily and documented his progress in his treatment chart. QMHP-C assisted with reducing anger-provoking situations through self-talk and role play scenarios in order to prevent escalation and dealing with anger assertively. QMHP-C continued to assist Jeremiah with managing anger and frustration by reviewing and role-playing effective coping skills. Intervention: QMHP-C helped Jeremiah problem solve when he began displaying negativity towards himself and others. QMHP-C modeled proper positive behavioral expectations for the Jeremiah to encourage additional layers of support and understanding of rules and expectations. Although Jeremiah needs improvement, QMHP-C commended Jeremiah on his efforts to partially maintain his ISP objectives as best as he could. QMHP-C observed Jeremiah dismissing without incident and preparing for the night in a positive manner, as witnessed by him asking for permission to go to his room after the session ended. Response: Jeremiah is more confident in the session, as evidenced by his assertive and proud tone when discussing parts of his day. Jeremiah have been doing a better job controlling impulsive and hyperactive behaviors, due to keeping his hands to himself and staying in designated area. Jeremiah reduces sadness and finds more excitement in learning about his mental illness. Jeremiah displays good participation in session. Jeremiah exhibits appropriate behavior in the home, as observed during the role play activity. Jeremiah understands that sessions are a privilege and he have to show good work ethic and follow along with the interventions. Jeremiah displays coping skills when faced with conflict due to deep meditation exercises. Jeremiah anger is moderate to intense, 2-3x per week. Response: Jeremiah is working on walking away and telling an adult before responding to anger provoking situations. Jeremiah understands that problem solving starts with thinking before acting. Jeremiah demonstrates problem solving by staying calm and using his senses. Jeremiah acknowledges expectations and reviews goals and objectives with counselor and parent daily. Jeremiah displays good social behavior throughout the session, as observed and supported by mental health professional. Jeremiah is reported building healthy relationship with his mom and performing a lot better emotionally and verbally.
